Insurance Services may be covered in full or in part by your health insurance or EAP benefit plan. Please check your coverage carefully by asking the following questions:
What kind of mental health insurance benefits do you have?
What is your deductible and has it been met?
How many sessions per year does your health insurance cover?
What is the coverage amount per therapy session?
Is approval required from your primary care physician?
What is your out of network benefit?
I am a provider with Anthem Blue Cross-Blue Shield, Unitedhealthcare/Optum, and Cigna With other insurance companies, I am considered an out-of-network provider. If your insurance will cover an out-of-network mental health provider, I can prepare a monthly billing statement (superbill) to submit to your insurance company. You will be required to pay me directly for each session and you will be reimbursed from your health insurance company. Be aware that your insurance will likely require that I provide a mental health diagnosis and sometimes a treatment plan.
Reduced Fee Reduced fee services are available on a limited basis.
Payment Cash, check and all major credit cards accepted for payment.
Cancellation Policy If you do not keep your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session. For medical or emergency situations, this policy can be waived.
